  • What is the Dental Loyalty Program?

    The Dental Loyalty Program is a special program designed for our patients who do not have dental insurance. The Program offers considerable savings for dental care.

  • What are the benefits of the Dental Loyalty Plan?

    The annual membership fee entitles each member of the plan to the following preventive dental services at no charge:

    • Prophylaxis (cleaning) - Biannual.
    • Periodontal maintenance is covered (as an alternative benefit to prophylaxis) - Biannual.
    • Any additional dental procedures are eligible at 15% discount.
    • X-rays as needed during Prophylaxis visit (includes full mouth series).
    • Diagnosis and treatment planning.
    • Oral Cancer examination.
    • Fluoride treatments.
  • What Are The Advantages of our program over dental insurances?
    • No yearly maximum for benefits
    • No deductible
    • No waiting period
    • No exclusions of pre-existing conditions
    • No claim forms or other “red tape”
  • Are there any restrictions?
    • Dental Loyalty Program is not offered to patients with dental insurance.
    • Dental Loyalty Program cannot be used in coordination with any other discounts, (e.g., Senior Citizen discount).
    • Dental Loyalty Program membership is superseded and voided by enrollment in other dental insurance without refund.
  • How do I enroll?

    Complete the attached application. Mail it with your check or credit card number to the office. If need immediate dental care, call our office and bring your application with you.

  • When will benefits begin?

    Immediately upon registration.
